What Do You Want To Learn About In Future

Programmed Shared Medical Appointments

A programmed Shared Medical Appointment (pSMA) is a group-based GP Consultation, where a small group of patients with a similar chronic condition, such as Type-2 Diabetes, meet with one of our GP’s at the same time.

What sets a pSMA apart from a standard consultation?

  1. Planned & Structured (“programmed”)
The appointment topic is planned ahead of time, and patients are able to submit their questions before the appointment to ensure they are addressed by the GP.

  2. Shared Visit
Appointments are facilitated in small-groups, ensuring everyone hears the same content and has an opportunity to ask questions and learn from the questions and comments of their peers.

  3. Medical + Educational Focus
These appointments combine both clinical care with group education, providing patients both knowledge and tools to improve self-management.

  4. Chronic Conditions
pSMA’s are a powerful platform to support education and empowerment for chronic or lifestyle-related conditions such as: diabetes, weight management, hypertension, menopause, osteoporosis and heart health.

Key Benefits of attending a pSMA include:

  1. More time for education and coaching than in a shorter one-on-one visit

  2. Peer learning and support from others with the same or similar condition

  3. Motivation and Accountability including goal-setting and follow-up check-ins that help you stick with healthy changes.
